Plan updates for 2023

The following 2023 plan changes are effective Jan. 1, 2023:

• Diabetes prevention program: Moda Health is offering a diabetes prevention program through Livongo. The program includes an advanced smart scale and app, unlimited one-on-one expert coaching and all-in-one weight, activity and food tracking.
• Increase orthodontia lifetime maximum: Delta Dental is increasing the orthodontia lifetime maximum from $1,500 to $1,800 for full-time PPO and Premier plans.

Get the most out of your health benefits

When you choose a Moda Synergy plan, you’ll get:

• OHSU Health — A wide selection of high-quality in-network providers in Oregon, Southern Washington and Idaho, including OHSU Health, Legacy Health, Adventist Health and Hillsboro Medical Center.
• No referrals needed — This means you don’t need to get permission from your primary doctor or health plan for an in-network specialists or alternative care providers.
• Alternative care — Take advantage of one of the largest networks of alternative care providers (including licensed massage therapists!)
• Moda 360 Health Navigators — Get your own enhanced support team when you choose both a medical and dental plan with us. Your personal Health Navigator can help you get great care from head to toe.
• Member Dashboard — Your own personal website where you can view your benefits, access resources to help you manage your health and chat with a Moda 360 Health Navigator.

With a Delta Dental plan, you’ll get:

• In-network preventive services — This includes cleanings, which are covered at 100% and don’t accrue toward your plan maximum. This means that your dollars go further for basic and major services.
• Access to the nation’s largest network of dentists — Gives you more choices for nearby dentists wherever you go.
• Health through Oral Wellness® program — These enhanced benefits provide qualifying members with more cleanings, fluoride treatments, sealants and periodontal maintenance during the plan year.

Take your health assessment

Don’t forget to take your health assessment to participate in the Health Engagement Model (HEM), through your Member Dashboard account. A health assessment is important for knowing your current health status and learning what steps you can take to improve your health.
