Ask your dentist about extra dental benefits

You can now talk to your dentist about Delta Dental of Oregon’s Health through Oral Wellness® program. The program offers extra dental benefits to Moda members who have a greater risk for oral disease.

Start the conversation

Your eligibility for the program is determined by a clinical oral health assessment performed by a registered dentist.

You should have received a flyer and card in the mail to discuss the program at your next dental appointment. The card provides important information about how your dentist can register for the program.

See if you qualify

Follow these simple steps to get started:

1. Talk to your dentist about the program. If they are not registered, ask them to call our provider line at 844-663-4433 to learn how to sign up. Once registered, they can perform your oral health risk exam and let you know if you qualify.
2. Before seeing your dentist, you can take a free oral health risk self-assessment. You can choose to share your oral health risk self-assessment results with your dentist to start the conversation.
   Please note: a self-assessment does not qualify as your clinical oral health risk exam. Only a dentist registered with the program can perform your clinical assessment.

With extra dental benefits such as cleanings, fluoride treatments, sealants and periodontal maintenance, you can:

• Learn how to achieve and maintain optimal oral wellness
