Get discounted rates with in-network eye doctors

Did you know that if you have a vision plan with Moda Health, that plan is not tied to your Moda medical plan’s network?

In other words, if you are enrolled in a coordinated care model (CCM) medical plan, which uses either the Synergy or Summit network, you can still use a vision provider in the Connexus Network.

With your Moda vision plan, you can see any vision provider that we are contracted with, and will receive a discounted rate for their services.

Find an in-network provider To find an eye doctor in your network, you’ll want to use Find Care, our online provider search tool. Just follow these easy steps to locate a contracted provider near you:

• Log in to modahealth.com/mymoda.
• Log in to your account. If you don’t have an account, you can create one there.
• Choose Find Care
• Select Vision under the Type of Search drop-down menu
• You may search by provider name or location
