Plan highlights

The following benefits and requirements are part of Moda’s 2018-2019 health plans:

Delta Dental PPO providers required — Members enrolled on our Delta Dental Exclusive PPO plan must use a Delta Dental PPO provider in order to receive benefits. Out-of-network benefits are not covered with this plan.

OEBB Moda members receive the Virtual Visits benefit for a $10 copay, which is not subject to the deductible. Members on the Evergreen and Fir plans will have a $10 copay after the deductible. To learn more about our Virtual Visits, log in to your myModa account and see “Virtual Care” on the home page. To schedule a Virtual Visit, go to ohsu.edu/virtualvisits.
